iMPROve Health brings deep expertise in chronic disease management to support better health outcomes and a more effective healthcare system. Our work demonstrates our ability to lead high-impact quality improvement initiatives focused on education, provider support, and sustainable interventions.
In partnership with the Michigan Department of Health and Human Services (MDHHS), we help plan and facilitate the Michigan Improving Cardiovascular Health (MICH) Collaborative. This statewide initiative promotes best practices and enhances cardiovascular outcomes through collaborative efforts between clinical and community settings.
Self-Measured Blood Pressure (SMBP)
Our team has successfully supported healthcare providers in implementing SMBP programs, which have been proven to reduce the risks associated with hypertension. Our experience includes:
- Developing blood pressure cuff loaner programs
- Identifying eligible patients for SMBP
- Assisting clinics in collecting and integrating patient-reported blood pressure data
These efforts improved medication adherence, patient engagement, and health system processes.
