iMPROve Health is a member of the Superior Health Quality Alliance (Superior Health), an eight-partner organization focused on improving care for Medicare beneficiaries. We collaborate with patients, clinicians, healthcare organizations, and communities across Illinois, Indiana, Michigan, Minnesota, Ohio, and Wisconsin to support the Centers for Medicare & Medicaid Services’ (CMS) quality improvement objectives.
We have been participating in CMS’s quality improvement efforts since its inception, with the 12th Statement of Work (SOW) complete in 2024. Through this work, Superior Health achieved significant milestones, including:
- 58% relative improvement rate in beneficiaries with diabetes
- 24% relative improvement rate in screening and diagnosing chronic kidney disease
- 86% reduction in infection control citations among long-term care facilities
